Nobody want to go on dialysis, and our goal as kidney function worsens, is to see if you are a candidate for a kidney transplant.  After having had a kidney transplant, the transplant center usually follows the patient for about a year.  After that point, the local nephrologist assumes care of the various aspects of post-transplant care.
